§1

General provisions

1. These terms and conditions set out the rules for using the services provided by the Psychological Centre Sztuka Harmonii, with its registered office in Gdańsk.

2. The service provider is the Psychological Centre Sztuka Harmonii, address: Piekarnicza 5, 80-126 Gdańsk.

§2

Scope of services

1. The Centre offers services in the fields of psychological consultations, psychotherapy, psychological diagnostics, sexology, psychodietetics, psychotraumatology and psycho-oncology.

2. Services are provided in person at the Centre's offices and online via video-call platforms.

3. A detailed price list of services is available on the website under the Services tab.

§3

Booking appointments

1. Appointments can be booked by phone on 732 059 980, through the ZnanyLekarz portal or via the contact form on the website.

2. Appointments are confirmed by phone or by SMS/email message.

3. The patient is required to attend the appointment at the agreed time.

§4

Cancelling appointments

1. An appointment can be cancelled free of charge up to 24 hours before the scheduled time.

2. If an appointment is cancelled less than 24 hours in advance, or if the patient does not attend the appointment, the Centre reserves the right to charge the full fee.

3. Appointments can be cancelled by phone on 732 059 980.

§5

Payments

1. Payment for services is made before the appointment or immediately after it ends.

2. Accepted forms of payment: cash, bank transfer, BLIK.

3. A VAT invoice is issued at the patient's request.

§6

Confidentiality

1. The Centre's specialists are obliged to observe professional secrecy in accordance with the Psychologist's Code of Ethics of the Polish Psychological Association and the applicable laws.

2. Information shared during appointments is not disclosed to third parties without the patient's explicit consent, except in situations provided for by law.

§7

Final provisions

1. The Centre reserves the right to amend these terms and conditions. Patients will be informed of any changes on the website.

2. Matters not covered by these terms and conditions are governed by Polish law.

3. These terms and conditions come into force on 1 January 2024.
